There are two types of customs forms and you can obtain these from your post office or generate them online.

Customs form PS Form 2976 - CN22 (2976 for online) is for a package with a maximum weight of 4 lbs and value up to $400

If the weight exceeds 4 lbs or value exceeds $ 400 you will need to complete PS Form 2976 –CP72 (2976-A for online)
